Investment Club Annual Verification Report

To protect your members' investment club accounts from possible misappropriation, and to ensure adequate and accurate records of your investment club's transactions, NAIC recommends clubs appoint a committee of club members to verify the accuracy of the club's records each year.

If your club carries the Fidelity Bond available from NAIC, this verification should be completed by March 31 each year. The report of the club verification committee should be signed by all members of the committee and filed with the club secretary as a part of the minutes of the April, or earlier, meeting. Do not return this form to NAIC. Your Fidelity Bond requires this report be available for inspection. Claims under the Fidelity Bond must be filed within one year of the loss.

See Chapter 9 of NAIC's Accounting Manual (1991 Edition, Revised 1995) for instructions as to how to conduct the verification of club records.

The undersigned have conducted a verification of certain figures on the books of our investment club in accordance with the procedures outlined in Chapter 9 of NAIC's Accounting Manual for the calendar year 19__.

All deposits shown as paid-in by members have been deposited in the club's account.

All dividends due the club have been recorded in the club's books as received or receivable.

The club's cash on hand has been verified with the bank and/or broker statement.

All of the club's securities are listed on the attached broker's statement, or are in the club's safe deposit box, and have been counted. Where stocks are purchased in Dividend Reinvestment Plans, club books and the plan agent statements are in agreement.

A statement has been provided to each member showing their total valuation units in the club, share of club dividends, interest, short-term and long-term capital gains, and expenses for the past year.
